Founded by health care professionals and social workers, it is a one-stop shop for families and other caregivers looking for long-term care options for their loved ones. CareConnect takes a uniquely personalized approach to match seniors with care providers and helps the provider selection process and the transition to care go smoothly from first visit to the final paperwork.

CareConnect is paid and supported by the care providers in our network, which is why our service is provided to seniors, their family, and those in need of care at no charge.

CareConnect advocates do more than just help find care; they support seniors before, during, and after the senior transitions to care in order to help guide and prepare those in need of care. CareConnect advocates also gather feedback from our seniors to better understand their needs so that we can advocate for our seniors in situations where they need more support.

CareConnect works with a large network of providers across California. CareConnect helps educate those in need of care by assisting with the following types of providers: Adult Day Care Programs, Home Care, Independent Living, Assisted Living, and Memory Care, Continuing Care Retirement Communities, Adult Residential Care Homes, Community Care Foster Family Homes, and Skilled Nursing Facilities.
